It's also happening to me. I can play for exactly 3 minutes before it crashes (on the Valve Index), probably due to this memory leak again.
As for the controllers, it's a separate issue but since the Minecraft folder has restricted access, SteamVR can't reach it, It would be necessary to add support for the Valve Index controllers. For now Valve Index players have to use an Xbox controller.
