You don't even need a firework to trigger this bug. Just look straight up while flying the elytra and you'll lose the connection to the internal server as soon as you start falling.

This glitch is so annoying. I can't access the world anymore until a fix is released. Please release a bug fix version ASAP.
You should have a short timespan in which you can do something. If you have access to commands, you can enter /gamemode 3 into chat from the clipboard. If not, you can try to open to LAN to get command access, you can try to steer down very quickly (maybe even with a rocket) or you can use a program like NBTEdit to set your motion to 0 0 0.
How about they just remove the survival players can't fly rule? Then you wont keep getting kicked and this kind of thing can't happen again because it has happend before with servers where you'dd just get kicked for flying.
It's there to prevent hacked clients from just flying around the world like in Creative. It's one of the things that prevents cheating. It's pretty difficult to figure out when a player is moving properly with an elytra and when he's using cheats. Don't worry, it'll be fixed before the 1.13 release.

Command to summon Phantom:

Basically, if you summon a saddled pig riding a Phantom, start riding the pig, switch gamemode to Survival, and then /kill @e[type=!Player], instead of falling down as expected, the game crashes or kicks you out for "Invalid move packet received". Tested in 18w09a.

Another thing: When I had this, I got kicked even though I was in the Nether.
BTW, a way I've found to reliably trigger it quickly is get flying, and spam rockets while "snapping" your view up and down quickly.
