Stronghold did not generate in Survival mode (4873613235747845350) Tracked stronghold with eye of ender to location, dug down, and stronghold was not present. Recreated the world in creative mode and the stronghold was present at exact location where it was not present in survival.
Ensured I was using exact xyz coordinates in each environment

No. Structures were set to generate. I have been to a nether fortress, numerous mine shafts and villages already.
I can confirm something similar:
Generated multiplayer survival map on 18w15a (using minecraft_server.jar)
/location Stronghold would lead me to 8, ~, 8
No stronghold there
Updated to 18w16a
/location Stronghold would lead me to 1000, ~, -1333
No stronghold there
... restarted the game a few times ... (still on 18w16a)
/location Stronghold would lead me to 8, ~, 8 again
Still, no stronghold there
When I created a new singleplayer survival map using the exact same seed on 18w16a the `/location Stronghold command would lead to 1000, ~, -1333 again, and the stronghold would actually be there.
Is there any chance this can be fixed going forward from now?
Or do I have to generate a new map if I want to play the stronghold?

It seems this has been resolved, as of 18w20b. I just started a single player world and tested the locate command and it worked and the stronghold was at the location it said.
As a further test I logged into our playtest MP server which had previously produced bugged answers to the locate command. Strongholds did not generate near the locations suggested by Amidst or other mapping tools. As of 18w20b I can confirm that Strongholds generate where expected and that the locate command works, even on worlds imported from 18w19a. Please note that when we found that the locate command was bugged and Strongholds were not where they should have been we took care not to generate chunks where Amidst reported Strongholds should have been. I would suggest that users who have imported worlds from before the bug was resolved will have to purge chunks to make them regenerate to ensure that the fixed world generation works correctly.
This issue is not resolved.
As Christopher Martin has said, if you make a Survival world with cheats enabled, strongholds do spawn. However, if you re-create that same world in survival with cheats off, no strongholds will be present.
The testing I've done has strongholds only spawning in Creative and Survival with cheats, not vanilla Survival. I'd note that this issue needs to be fixed if proper testing of the End is to be done, since bugs can apparently only apply to certain game modes and not every game mode.

Don't you have paid testers? It's a huge hassle to make the paying customer go to so much trouble as to generate the world 4 times and get to the stronghold, especially in survival with cheats disabled. It's a solid hour's work. And there are no external editors that work on the snapshots yet.
Either way, this happens also on seed 7139399895245564268, Creative, cheats enabled. No table, sorry.

@unknown: If you don't like to test a snapshot (= alpha version) then simply don't do it.
You have accepted in the Launcher the disclaimer saying that snapshots will yield bugs, errors, crashes and corrupted worlds.
So please don't complain about getting bugs.
Pardon? I never complained about getting bugs. I think that was rather clear. I complained about the request to invest so much time into researching a particular bug and preparing that table. As I've said, that sounds like a solid hour's work that should be done by paid testers.
I just wanted to ask for help with finding out what's the cause of this issue because I wasn't able to reproduce the issue myself. If you don't want to help, that's fine.
Just to be clear, this tracker is almost completely run by volunteers (like Kumasasa and me). Mojang doesn't pay anyone for bug finding or testing, that's always done by the community.
That's very surprising to hear. They probably should.
Either way, I think I might have some new information. I believe strongholds are actually generated but sometimes their location is reported incorrectly by the /locate command, which always claims a stronghold should be at (8, ~, 8) — the exact location in one of the above comments. I tried reloading the map where I initially encountered the poblem and it corectly told me where to find a stronghold.
Oh, and another thing. It's not just the /locate command. Throwing ender eyes also lead me to that wrong coordinate mentioned above the first time I tested.

As described, /locate and Eyes of Ender direct us to [-40, ~, -1640]. Went there in Survival using Eyes, no Stronghold. /locate, no stronghold ON THE SERVER.
I started a new world single-player using the same seed, verified it is identical to our server. /locate worked just fine, a stronghold is at the expected location[-40, ~, -1640] so the stronghold failed to generate. We were getting the MC-129136 game crash bug "exception ticking world" while trying to use Eyes of Ender to get to the Stronghold. I suspect that game crash prevented the stronghold from spawning.
The only current work-around we can see at the moment is to copy the newly generated region world in single-player over to the server.
EDIT: My other players traveled far enough away to locate another stronghold location. /location and Eyes of Ender work correctly to find it at [5288, ~, 6328]. We did not crash the server this time. My 'guess' is that something triggered the MC-129136 crash before the Stronghold could be generated causing generation to fail. The MC-129136 bug may or may not be related to Stronghold generation. We did not trigger the crash, so the second Stronghold succeeded to generate on our server. We are going to live with this Stronghold since trying to replace the first one isn't without risk.

Did you disable "Generate Structures" when creating the first world?
