Attempting to place torches from the offhand often results in the torch immediately disappearing and visually disappearing from the inventory. Switching back to the main hand replaces any "lost" torches in the inventory, and allows regular placement of torches.

Even if you searched "torch offhand", you could still have found the duplicates of MC-132635. At least you would have known that this had already been reported.
