Have tried in creative initially, tried survival afterwards to see if it only affected creative, also an issue in survival. I can create blank banners and that's about it.
Have tried basic problem solving (using any combination of colors/ingredients/patterns, restarting the game, reinstalling the game, restarting the computer, etc.)
Banner patterns are applied with the loom block now
