Hello,
Every time I dismount a mine cart in the Nether (pressing Shift), I fall down into lava. I suspect the bug causes the character to be misplaced a few blocks away and, if on a tunnel above lava, causes the character to fall below the tunnel.

We're actually already tracking this issue in MC-148869, so I resolved and linked this ticket as a duplicate.
