The bug
When exiting a minecart in a 2 block high space with a ceiling on the third block, the player would be placed a block into the ground if exiting the minecart by quickly tapping the dismount key (shift). This only happens when the cart has derailed, but can happen in all dimensions.
How to reproduce
Create a 2 block high room, at least 3x3 from the centre of the cart to remove all other dismount spaces
Place a minecart onto track
Break the track beneath the cart or ride the cart off of the track
Mount the cart
Dismount the cart, this only needs to be done quickly if the dismount key is bound to the same as the "sneak" key [Default: 'Left Shift']
Completing this steps will result with the player being placed in the ground.

Reopened. This issue didn't happen in 1.13.2, so the issue isn't quite the same. Can confirm.

I've just encountered a variant of this in 1.14.2 (but not in 1.14 or 1.14.1) with a minecart that is attached to a rail - when the minecart's chosen exit tile (N/S/NE/SE/NW/SW/E/W) is exactly 2 blocks tall, (i.e. the ceiling is solid), you take suffocation damage and land in the target tile, but if the selected exit tile is 2 and a half blocks tall (i.e. the ceiling is a top-slab), then you clip directly through the floor of the tile the minecart was on without taking any suffocation damage.
This one was quite nasty for me, since I encountered it in my Enderman farm at Y=1 and it spontaneously dropped me into the void.

I also get this bug, but I have a slightly different experience:
Playing on Realms 1.14.1
I have a nether highway (I'm far from spawn) that I use frequently, and if I (or anyone else) were to dismount the moving cart (it can be stationary too) they would fall into the block BENEATH the mine cart's track... which is all good and fine, if it wasn't for the fact that the highway is only 1 block thick, so everyone just falls through the floor and dies to lava (losing countless items/levels).
Not to mention, if a mob is BESIDES the tracks, and they're getting pushed or feel like pushing on you while you're passing by, you get pushed out of the mine cart and plummet through the floor to your fiery death.
This bug occurs in over-world and the nether (we haven't been to the end as of yet, so no idea if it'd work there or not). And it happens 100% of the time.

I can confirm this is happening for me as @@unknown described it. I have seen this in 1.14.2 and also 1.14.3.

This is also happening in 1.14.3 when being ejected by an activator rail. I have a 2.5 block high ceiling to protect against Enderman in The End. I've got the wiring for the activator rail in a 1-block high space under the minecart tracks and the majority of the time it teleports me to this area in the swimming position.

Can confirm the same as @Chris Andrews described, but also in 1.14.4.

I can confirm it in 1.14.4. It is deadly in The End dimension.

Still in 1.15.2. According to Mojang, 1.15.2 will likely be the final 1.15 version, but this bug urgently needs a fix.
