Phantoms aren't spawning at night after no sleep for 3 nights.
After no sleep for 25 nights now, still none.

unable to reproduce. Phantoms spawned for me at day 3 of the world with hard difficulty.
Requirements to spawn phantom are:
domobspawning true,
doinsomnia true
world overworld
no blocks above head of player
night time
player above Y=63
time since rest 1 hour real time.
Check if all requirements were met on your test setup.
What is /gamerule doInsomnia set to?
