When entering a nether portal from the overworld, you spawn in a different location in the nether that doesn't match with the portal. The video was recorded in a 20w10a server, with a nether hub portal, so It's quite high.
How to reproduce:
1- Place a portal in the overworld
2- Cross the portal
3- Go back to the overworld
4- Cross the portal again, spawning in the nether under the portal, and quite far.
This bug does not always happen, I do not know when it does and when it does not, but I've attached a video and some screenshots.

I want to add to this that if you use commands to create a nether portal that is smaller that what's possible in survival, it no longer connects properly. It puts you below the portal from what I can tell.

If I hadn't broken that block at some point, that poor sheep would have died! Not to mention if I had been in survival, or worse, hardcore, I could have died (apart from being unable to make a portal that small of course). I've tested multiple times on different worlds, broken and rebuilt portals, and resized portals. It seems like as long as they can be built is survival, so default and larger, they work. It's when they are smaller than the default portal we all know and love that things no longer line up. (20w12a)
Ok, so I learned a little more in 20w13a. It seems like a single portal block works, however small ones like the one above or on the left of this picture don't work (2*2 portals also don't work to my knowledge). Custom shaped ones like the one below also don't work. (ignore the little basalt generator on the right.)

Could you attach a screenshot of the portal on the Nether side? This sounds a lot like MC-121099.
