The main problem is that my friends and I use a realm and sometimes when we go through the end portal in the stronghold, we enter the End and fall all the way into the void without touching the obsidian platform. So we fall off as soon as we enter the end. I don't know why this happens and it only happens sometimes. But we have had to restore more than five backups to prevent lost items.
