I was playing around in a vanilla survival world (other than OptiFine), jumped off a mountain into water (I know for sure I hit the water, not a lily pad or ground). A slight, maybe half second delay occurred before the game said I died with a death message simply stating "death.fell.accident.water." Since I had the debug screen up, it clearly shows I was in the water, because the Y position was 62.12315! It should be noted that I had some lag at the time of my death, but there was no possible way I could've landed on land.

Optifine shouldn't have any effect on how you die. Another report of the death message was described here: https://bugs.mojang.com/browse/MC-195467
That report could also potentially be by someone using OptiFine, but I don't see how a performance-based add-on could add a possible death. I've played since 2013 and never seen this death message, so it'd be nearly impossible to recreate in a completely vanilla world.

Any non-standard client/server/launcher build needs to be taken up with the appropriate team, not Mojang Studios or the Bugtracker.
Any plugin or resource pack issues need to be addressed to the creator of the plugin or resource pack.
If you have problems on large servers, such as The Hive and Hypixel, please contact them first as they run extensively modified server software.
