I use the Optifine mod if that helps any, but it also affects my friends who do not.

According to NameMC, that user just recently changed their skin, so now you get the newer skin when you obtain their head. Is that what you're experiencing? The head in the screenshot looks to be correct.

"MHF_Fox" is not maintained by Mojang.

I would like to point out to Galaxy_2Alex that according to the Official Minecraft Wiki,

"Mojang Studios has created a number of accounts with specific skins, so that map makers can use common heads without the risk of someone changing their skins. These accounts have names in the format MHF_<_Name_>, for example MHF_PigZombie is the name of a Minecraft user with a zombie pigman head. MHF stands for "Marc's Head Format". There are also a few blocks and "bonus" heads, for more variety."

The very first sentence in the paragraph says that Mojang themselves made these so as to avoid skin changing.

But the Fox was never part of this, and users are able to create any "MHF_<name>" account by themselves. Any request to add more variety to these heads are a feature request, not a bug.

According to that same page, however, MHF_Fox is not managed by Mojang (it is not included in the list of accounts). It is likely just a normal player who picked that name.
