Anvil destroys item when it breaks

I was repairing a diamond axe on an anvil that broke upon applying the repair. The item was destroyed along with the anvil. I had 2 anvils on top of the one that broke that popped into entities (Not sure if this is related to the issue but thought the whole picture could help)
