NOTE - My issue affects all versions! Not just the snapshot.
Due to the final upload limit, I'm having to link to my video showcasing the issue: https://youtu.be/5Ssg2H1Z6EE
Specs of my PC are in the F3 screen I show.
For more context, I've had this survival world for 2 years... and the lag has progressively gotten worse. I thought it was entities causing lag but clearly it's not (I show that in the video)
I fly about 500 blocks away from my home base, and boom... FPS goes back up. Not a clue what the issue is.
I'm more than happy to provide you with more information... I don't experience game crashes or anything - it's literally just constant FPS drops... on SMP servers I get 200+ FPS easily... this issue seems to be very specific to this world so I'm not sure what's causing it.
Thank you for taking the time to help!

There are over a thousand particles being processed in your main area, whereas it drops down to 20-30 when you're out of that area, so the FPS drop is to be expected with that number. So I don't see anything abnormal here.
You should figure out what's the source of this many particles is and see if the fps situation improves when getting rid of them.
Also your randomTickSpeed is set to 30. You should set this back to the default (3).
Appreciate you looking into this. I had no idea my tickspeed was changed, I'll revert that now.
As for the particles causing the lag, is there a way to identify which particles are causing lag or is it just trial and error to resolve this?
Yet again, thank you
I want to update this...
I've done tons of testing on copies of my world... something isn't right at all.
As I said the first test I did was killing all entities. That helped a bit, but very little.
I've now tested particles causing lag... I got it to around 100 at all times using world edit and removing things like torches, nether portal, and many more things that produce particles. Again, this helped more than the entities - but not enough...
I've tested different Minecraft versions on old copies of my world (I have a backup from every released version including snapshots) still nothing while testing that.
I've tested Optifine 1.16.4... it increased by maybe 30fps. Please bare in mind that I'm used to playing on 250+ on brand new world or servers, so it does feel horrible to play on a world that I spent so long on!
I know 100% it's nothing to do with my PC specs, so I'm literally at a loss for ideas as to what to do... spent the last 8 hours on this trying to fix it and barely got any results.
At the time of this post, the video is still processing. I'm assuming by the time someone gets assigned to this, it will be ready to view.
