[MC-219074] Stronghold portal sometimes sends you on the outer end islands

While playing on my SMP server i went to my stronghold so i can gather end stone for a build. it timed me out and then after reconnecting i ended up teleporting 1500 blocks on the Z axis away from the center of the end.

Please provide a video with the debug screen enabled (hit the F3 key) demonstrating the issue.

Confirmed its existence, if you disconnect while entering the end portal it copies over the overworld coordinates and spawns you wherever your strong hold was in respective dimensions
