After experiencing Lag spikes on our server(vanilla) since we launched 1.17 (new world) we finally had an idea and tested it.
The Idea is that opening item containers causes a lag spike as the server gets the information for the items within. We've noticed similar things with all blocks that can have items in and villagers but we didn't test everything.
Test: everyone online, 7 people, stood in the same area. Lag was extremely low. We then simultaneously opened an item container. 6 Shulkerboxes and 1 barrel. All containing items. We closed the inventory as soon as it opened and re-opened it.
Extreme lag started immediately.
We lost over 2k ticks and the server didn't even calculate the loss until we had stopped.

I’m getting the same thing. Even things like furnaces and the villager trading interface. Really frustrating, I know it’s keeping some people on my server from playing at all.
Our server did a test, 7 people online, all in spawn chunks. I included the debug files for you to look at. I tried to take about 12 debug files, but the server was running to slow to save all but 2 of them. I've included the logs here.
Also experiencing this issue, Massive like 1-2 seconds of just frozen minecraft everytime someone opens an inventory
Does MC-178267 describe your issue?
I've had the same issue on the server I play on. It seems the spikes in tick loss happen at every item menu open (except one's own inventory)
