[MC-245439] Doors in strongholds sometimes generate without blocks underneath

When a stronghold is generated intersecting a ravine, some doors will generate without a supporting block underneath.

Second one, except that MC-216881 is also a thing; so I'd suggest resolving it to that. The fact caves intersect has more recently been considered a bug than intended design.
