If you try to place a block in your main hand at the same time as switching it to your off-hand, you will not place the block, despite it being a valid action in either hand. This causes a very subtle lighting glitch as well.
If you have a block in either hand, it will appear to place the one that was in your main hand and make that sound, then immediately switch to the other.
The lighting glitch seems to have been fixed as of one of the 1.20 snapshots (probably whichever one changed the lighting engine) but the rest of the bug is still valid.

Honestly it doesn't really matter if you aren't able to reproduce it in survival if it happens in creative, it still is valid. Tried to upload a video of the "fail" occurring but I am getting error with the bug tracker
To clarify my previous comment, the behavior of the blocks appearing and quickly being replaced is both present in survival and creative. My point was that the item is not consumed in survival, meaning there is no loss of items.
This seems to be working as intended. Trying to reproduce this in survival I am unable to "fail" one of the blocks from being placed. While visually you see one of the blocks appear for a brief moment before the next is actually placed, the item of the first block is never consumed.
