Okay, so if you create a world with the world type set to single biome. Then set doMobSpawning to false, mobs will still spawn. It still happens if I try /kill @e, the mobs come right back.
I feel this isn't intended.

doMobSpawning only prevents natural spawning. Mobs generating with terrain is not the same thing.
