Almost every time I place a block,torch,pick up item out of a chest,etc. It will place the block twice it it can be placed again. I have no idea if its my mouse or the game itself.

Lag, I would say 99.9% it is lag. especially if on SMP but it can still happen on SSP

I have the same issue. Placing blocks and opening or closing doors or gates is often repeated due to visible lag (not a mouse issue). The lag happens exactly at the moment I place a block or open/close the door, so there's no avoiding it. There's a portal chamber and a multilevel basement beneath the area the lag occurs. I've seen someone mentioning on reddit, that due to a bug, every time you update or place a block above an empty chamber (room, ravine, cave etc.), the game updates the lighting in that chamber, and this creates lag. I don't know if that's the case, unless someone else says so, I'll stay at "No".
I play on a LAN SMP server (Minecraft v1.4.2, Oracle Java 1.7.0_9, Ubuntu 12.04, 2GB RAM, 1.5GHz Dualcore Atom) set up on a separate machine, with 2 clients connected, both having the same issue in the same area.
I have the same thing sice I updated to Windows 8. Worked perfectly before, but now most of the time multiple blocks are placed on one klick. It can't be my system (high end hardware, 32 GB RAM). Someone in the forum said i should install the official Logitech mouse drivers. That didn't help... Different mouse, same problem. Many people seem to have this problem and it keeps me from playing a lot since updating to Windows 8... It happens in Single Player, I don't play much SMP these days.
SMP and SSP are pretty much the same, cause SSP is simply an local server + client. Maybe you have to update your sound engine jar. i heard from many people that they were having issues which are fixed after updating sound engine.

I still can't believe this has not been fixed in a patch. This issue has been around since...I don't even remember how long now. And I know every one keeps saying it's leg, but I've seen then happen on high end computers on single player mode, or even when a server is running at peak efficiency. For me, this issue is actually worse on my solo game, and I waste so much time removing blocks I did not want. There are areas of my base I have not worked on in several months, /small/ areas that are minimally lit, cause I don't like the look of torches everywhere, and it happens with almost every single click. This seriously makes any kind of project extremely frustrating, and so far, has been my only real complaint with Mine Craft...and has become more so the longer it has taken to resolve it. Even after upgrading a computer, this still happens.
I dont think its minecraft itself, cause nobody else has talked about such a bug. Maybe its a corrupt world file... Have you tried to create a new world ??
