textures disappear or change color

I play for a while and the textures of my items disappear, then the blocks and everything disappear, sometimes the mobs turn dark and the chests too, sometimes also the textures of the items the font and the character become totally one color, I have to leave the server and exit minecraft and enter again to fix it which makes it unplayable
