This was on a server, and I don’t know if that has anything to do with it, but it is a possibility. I haven’t been able to reproduce it, but, this could work in vanilla Minecraft to turn something into something else. People could lose netherite, diamonds, beacons, or other valuables due to accidentally doing this.

This is very likely a client-side bug/issue. If a torch was dropped, then the pickaxe is probably still in your hotbar, you just don’t see it. Clicking the hotbar slot in your inventory makes the pickaxe reappear.
