[Summary] instead it only teleports player when player pauses game via esc, opening a command block menu or any other way.
Ran it on vanilla as well, same effect so mods aren’t interfering.

Some more info (might be incorrect)
the player does actually get moved to the correct place however on the client it doesn’t get moved there every single time. For example in multiplayer you will see the other player (one thats being teleported to a spot) at the right place. You won’t be able to interact with entities or blocks further than the player reach.
Only goes for relative teleport (using ~ ~ ~), /tp to specific coordinates or entity works fine
