The problem I am having is that when I sneak / crouch and move forward it is really glitchy and no longer moves smoothly. (Game mode does not make a difference)

Yes, it is still happening. Is there any logs that I can attach to help out in debugging? Should I uninstall and re-install all versions of Java and Minecraft? Would that help try to find the bug?

This is still an issue. I have asked you if there is anything more I can give you to help resolve this.
It makes the game much harder to play and to be honesty because of this issue, I have stopped playing as it is just frustrating to deal with.
You might try rebinding the crouch function in Minecraft to a mouse button to see if it's a keyboard input problem. You could also try playing with the Windows character repeat settings in the control panel keyboard properties. Try setting the repeat delay to the longest and the repeat rate to the slowest (taking note of what they default to so you can set it back to what you're used to, there's no "default" button to reset them.)
Jon, those were good ideas, so today I went home and tried them.
You were spot on. Both my Shift keys and ctrl keys are messing up within the game (But work without issues on every application on my system/web). I tagged it to Z and had no problem. ugh, but I really like this keyboard too.
Before closing this ticket, I will attach another keyboard and re-test to make sure.
