I was experimenting a bit more and was able to reproduce the issue or a variant on it that can take over this ticket.
In some versions of the game I'm able to crash the game and in others I'm able to end up with a frameless window (no title bar with close/minimize/expand button).
If F11 works for you, use that. I used the Full Screen button in Video Settings to accomplish this:
command+control+F to enter native fullscreen

At this point, my computer screen is mostly black/or blurry. I press command+control+F one more time and the game exits full screen but has no window controls (close, minimize, expand etc.). The window is sometimes larger than my screen and sometimes smaller.
In prior versions (1.19) this crashed the game.
Actually, I think this issue has been resolved. I'm unable to make the game crash and while it's super glitchy while switching into Minecraft fullscreen it doesn't seem to cause any real problems.
When you said you reproduced it, what do you mean by "it"? Just going into the secondary fullscreen? I'm in 1.20.4
Is it called Minecraft Launcher on Windows?
I can also confirm this is fixed on my system as well, the icon is the correct dimensions and has rounded corners. I included cache reset instructions here.
Confirmed that this has been resolved. If you are having trouble seeing the new icon, the following command in terminal refreshed it for me:
rm -r /var/folders/*/*/*/com.apple.iconservices*; killall Dock;

is not part of this ticket and so is not related to those other things you're describing.
Since this ticket has ~150 people subscribed to it, I'd rather not extend discussion too much and spam people's mailboxes. This ticket is strictly related to the desync that occurs when an item is on a block edge, which has not been fixed. No repeated teleportation is needed to reproduce this issue.

The steps you suggested are not related to this issue. While it is true that what you suggested does show (possibly) incorrect behavior, that would be a different bug that's only mildly related to this one.
This bug concerns the client receiving incorrect coordinate information. The client is taking the correct action based on the incorrect data it receives. The item falling is merely demonstrating that bad data was received.

Your game, launcher or server is modified.
If you can reproduce the issue in a vanilla environment, please recreate the issue.
Any non-standard client/server/launcher build needs to be taken up with the appropriate team, not Mojang.
Any plugin issues need to be addressed to the creator of the plugin or resource pack.
If you have problems on large servers, such as The Hive and Hypixel, please contact them first as they run modified server software.

It seems like you have your server distance set to 5 chunks, which is why the fog is so close. Your client-side settings (what's shown in the video) won't go above the server side setting.
Java Edition does not have an experimental mode. Are you playing Windows 10 Edition (Bedrock) or are you playing Java Edition 1.18.1 Pre-release?
