Blocks When Mined Shoot Randomly Far Distances from Player

I was mining in general and noticed that when cutting trees, blocks seems to shoot really far away from player, as when I died, I found my items almost 20 blocks away from where I died . Then I was really surprised when mining precious iron on y9 that did not make it into my inventory. When I went back up to my base on y93 to unload, low and behold, an iron item block hovering outside my door. Thats a huge miss from y9 lol.

I do not believe this is an issue of it going into inventory, this is an issue of what happens to a block when it is mined. It no longer drops, it flies.
