when using the command

/setblock ~ ~ ~ ladder

the ladder has the "missing block" texture.

Do /setblock ~ ~ ~ minecraft:ladder

@unknown, that's the same thing.
@unknown, works as intended. You need to specify a sub-id for the orientation, otherwise it can't render correctly.

Place a ladder in creative, then press F3 and view what data value that ladder version has. Copy that into your setblock command

/setblock ~ ~ ~ minecraft:ladder 4
