Steps to Reproduce:
This seems to only happen when a creeper explosion happens in the vicinity of an extended piston; I fond this out when trying to farm Music Discs via pressure plate - piston trapping.
It appears that the extended piston is causing the crash. Possible duplicate of MC-58300
