Whenever you create a debug world, all blocks when broken get replaced by the block you just mined and don't drop their item (in survival/adventure), some blocks, which should break (aka buttons, levers, etc.) when you activate them the drop as an item, and also revert to their previous block state.

1. Create a debug world
2. Break a block, or click a button

this is probably because debug worlds are for making sure all blocks are working, not for playing on. It is essentially "view only"

I think this is intentional behaviour, I don't think your meant to be able to modify the world?
