Mining fatigue level 4 behaves like mining fatigue 5+

Without Mining fatigue level four I'm able to break blocks as normal, however with mining fatigue level 4, which according to the wiki ( http://minecraft.gamepedia.com/Effect ) is an 80% speed reduction, I am unable to break blocks.

This appears to be also apparent with level 3 and 2, but I have not tested them fully.

Confirmed. Works fine with level 2.

The bug isn't that you can't break in lv 4. However the mining cracks in the block doesn't show. But if you keep breaking it will brak at the end. Remember it's 80% so it will take like 10 sec for diamond pick on stone. But the cracks doesn't show. Thats the real bug.

With Mining Fatigue IV (= amplifier 3) it takes a lot of time, but the cracks will appear eventually.

Just tested and then isn't amplifier 4 supposed to be IV? Anyway I am apparently able to mine sandstone with diamond pick (without enchantments) with amplifier 4. And in amplifier 3 the cracks comes at the END of the mine. But it's supposed to show how far you are into the mining. Not that every single crack pop up in the end...

The valid range for status effects are Level I .. IV , that is amplifier 0 .. 3
Amplifier 4 is already outside of the valid range of status effects, you see that in your inventory no longer the level is shown, only "Mining fatigue".

The first cracks in my test came after ~ 20 seconds. Try to break sandstone, maybe it's material dependant.

Try it on a 20hz clock. Its still apparent in 1.8, as of recent testing.

20 hz clock ? What ? We're talking about a status effect, don't we ?

During testing, I was using a 20hz clock (the setblock / fill ones) with the command:

/effect @p 4 60 3

The issue may be that the client is refreshing the mining fatigue, so that you cannot mine anything as it keeps resetting your break.
