Put the four soul sand in a T like supposed to and put all three wither skeleton skulls on top but the wither doesn't spawn in like supposed to,

No its on hard mode, in the picture you can see the difficulty is set to 3.19 which means hard mode 0.00 difficulty means peaceful 🙂
Cannot reproduce. Did you create the wither statue in peaceful then go to hard? You have to create the wither on non-peaceful for it to spawn.
I had the same issue today on 14w31a. I tried spawning him four times, then moved him up one block and he spawned.
There must not be any tall grass besides the base block, see MC-60792
