An iron golem farm constructed such that all horizontal blocks outside the desired spawn zone are covered in half slabs andor glass, is not infrequently spawning golems on top of the half slabs. Note this is slightly different behavior from MC-3927, where a golem is spawned inside a half-slab and gets stuck. These golems are on top, can move around freely, and are vastly annoying. It doesn't happen often – maybe one every 2 hours or so. But that's enough to break the whole farm in under a day.
The problem is not new to 1.8.1. I've been facing this since I built the farm back in September.
The first three attached pictures attached show the golem spawned on the half slabs, including one close-up with F3 enabled showing the ground on which it stands, and one further back showing the overall structure (if it helps any). Also included is a shot of the upper deck of the collector which also spawned me a golem this morning (didn't get the pic in time before it walked off the edge. Like I said, they're not stuck in these slabs; at least I managed to solve for that problem) Update: two additional shots showing where a golem spawned on half-slabs in the upper deck village.
My map is available on request if that would help the defect team at all.

Happens without the mod, too. I'll be happy to re-add it. I only used the profile with a mod here to make sure I wasn't losing my mind entirely.
Added an image of a golem spawned on half-slabs without the use of any mods seconds before he dove off the edge.
This has to be a bug or a change in the golem spawn area.
My 1.8.x farm is 23 x 23 outside. 21 x 21 inside with a 3 x 3 hole. I've built this on a Vanilla server and 3 Spigot servers and can confirm there are no doors any where outside the farm, yet golems spawn on top of the villager holding cell (which is only 2 wide) and far outside the 16 x 16 area they are supposed to spawn in.
I've tried half slabbing and completely filling the area with fences and glass, in all cases golems spawn on top of or Inside the fences or glass. I'm going to try filling the area with stone blocks now and just leaving a gap for the doors to see sunlight.
My latest build is in Skyblock which confirms that there are no extra doors anywhere.
I tried filling the entire area around the villager holding cells with 1/2 slabs up to the height of the iron farm, (23 wide x 6 deep x 8 high only to have a golems spawn completely inside the slab, unable to move. I finally removed all the slabs and filled the entire area with solid blocks except for a 1 x 2 tunnel so I can trade with the villagers. villagers. This seems to have stopped the problem for now.
Can someone look at the source and let us know if the size of the spawning area has indeed changed?
Is this still an issue in the current Minecraft Snapshot 15w46a or later? If so, please update the affected versions in order to best aid Mojang ensuring bugs are still valid in the latest releases/pre-releases.
Ticket resolved as incomplete, because no answer in a reasonable amount of time (1+ year), if it still happens, please update the ticket.
Problem with Iron golems spawning on slabs, tried with glass blocks and slab on top of glass, and with no block and slab on top with half block space. (see pics)
Was hoping problems solved with 1.15 ... but no.
No mods, this ha been an issue for meg since 1.14,
Mojang: Any ideas how to fix?

Yep, about at the 2 hour mark this guy appeared in the upper deck (slightly different village design from the other one but still surrounded by only half-slabs and glass).
