The problem occurs only in the Nether in chunks around the portal. When a block is broken that chunk is rendered normally until a block in another chunk is broken.

Hmm, it seems very weird. Optifine's lazy chunk rendering can do this in extremely slow framerates, but please put more as what was expected, and tell in more details how that rendering error occured. Also what do you meant with "items and XP going through the portal"?
