Its basicly a HUGE chunk error, cant see blocks around the minecart, it fixes when you leave the cart, happens after riding around 50 blocks or so, (two block high gap) (underground)
The only way i have found to fix it, is to log off my server, and back on. takes me about 3-4 times per minecart ride that i need to log off.
