Game crashes upon login: Singleplayer & Multiplayer

Whenever I join a server or create a new world on single player and try to join it, my Minecraft crashes with the same error every time. I have tried to reinstall Java and Minecraft and did not seem to fix the issue. I believe the issue may occur with my Java version or LWJGL version (friend suggested that).
