[MC-98649] Certain Items Glitched Or Missing

I just started using Minecraft 1.9 and i have noticed that some of my items like signs and heads are glitched or the item is just missing. I have tried some things but it does not seem to fix it.

Is the resource pack you use outdated ?
Does this occur without resource pack ?

It happends without the resource pack. I forgot to turn it off when i took the snapshot.
