My minecraft launchs, I enter in a server, then he automatically craches.

I entered into minecraft, my pc isn't soo good so I have Optifine installed, I entered to a server, then poof... minecraft craches every time I try to enter, the game opens, but when I enter in a world, singleplayer or multiplayer, the game craches with this report:
