Died in survival mode with a map in my offhand slot, mapping out the terrain. After respawning and creating a new map, I can no longer place a map in the offhand slot. I click the map and try to click to place it but it does not release and the offhand slot stays empty. I am thus unable to view and map out the world.

You need to activate the map first by pressing use when it is in main hand. Then you will be able to use it in offhand.
