When entering or exiting a nether portal there is a chance that instead of being teleported to the corresponding portal in the nether, you will instead be teleported to a seemingly random location. This happened on a bedrock realm. I was the only player online at the time it occurred.
Examples:
I entered a portal at 54, 65, 209 overworld. I come out at -54, 60, 180 in the nether. However, the portal where i should have exited was located at 7, 110, 27.
I entered a portal at 500, 110, 0 nether. I come out at 750, 45, -234 overworld. However, the portal where i should have exited was located at 4000, 65, 0.
This happens both when exiting and leaving the nether. All portals where this happened already had linked portals in the corresponding places in the overworld/nether.

Hi I've just had this happen to me and my friend on the bedrock ps4 game. My friend made a portal at 767, 45, 2384. And then spawned in that same location in nether with no portal. And when i went through that same portal it sent her to 6152, 56, 19086 (in a cave, luckily it also added a portal so she could go back through the nether world) and it spawn me to where the portal should of been in the first place at 99, 32, 293 in nether.

I just had this happen to me. I have a nether hub that I built and am now 1500 blocks away from any nether portal I know of on this world, not to mention that I have no clue where in the nether I actually am besides coordinates

This has been happening to me a lot recently and is very frustrating as I often lose my items trying to make the journey of thousands of blocks back to my corresponding nether portal!

This has happened to me a couple of times now on a bedrock realm playing on the Nintendo Switch. Both times there were more than one player on the realm at the time.
First time I travelled from a nether portal in the nether to the overworld. Instead of exiting at my corresponding overworld nether portal, as I have done numerous times before, it sent me to a random location a few thousand blocks west over an ocean and I had to swim all the way back. Oddly, at the time I could not see the nether portal, obsidian, that the game created in this new location, but later another player spotted it floating up in the air above the ocean.
Second time I travelled from a nether portal in the overworld to the nether. Instead of exiting at the corresponding nether portal in the nether, as I have done numerous times before, it sent me to a random location east about 10,000 blocks away. This time I died trying to find a way home. Oddly, again, I could not see the nether portal obsidian that the game created in this new random location. However, as I started to mine a tunnel, the invisible obsidian blocks would become visible, one at a time, as I mined out a block of netherack adjacent to it.
