When killed by an Iron Golem and standing in the top corner of a room (only two blocks from ceiling so you can stand there) when you are killed by it you lose all of your inventory other than your armour and what is in your off hand. Items are not clipped into walls theyre just completely gone

it must of been that the items may had despawned as you might have been close to the items as they were loaded it takes the game 5 minutes for them to despawn
