It happens very less but when sometimes i play either in solo or multiplayer world and I use a tool from my hotbar generally sword and switch to other slots quickly for food or blocks the tool the tool automatically drops off fro the inventory... It happened many time to me and my friends as well... That caused loosing our tools and even lives while fighting to drowneds...
