Hi,
I have been having this issue lately. Most of the times I enter my nether portal or my end portal, it takes me to a completely different location than the actual portal in the other dimension. Sometimes even thousands of blocks. A friend of mine is having the same issue on his playstation 4 so i think it is a bug.
I hope someone can help me out.

I had that issue a few days ago on Xbox. I ended up in a underground tunnel under a desert temple 1000s of blocks from where I was meant to be.
