I will occasionally respawn at the wrong y level after dying, on both single player and multiplayer worlds, using a bed as my spawn point.
An example of this was when me and my friend were playing. My friend died, respawn at the wrong y level, in a dungeon, was bombarded by zombies, and died multiple times, always re spawning in the same dungeon.

Is your bed obstructed in any way? You might try making sure there is at least 1 block on each side/end of the bed and two blocks above the bed.
You can also attach a screenshot showing your bed location.
