Bone meal used on grass blocks only generates one grass/flower

Summary:
Bone meal is not working properly. When used on the ground, only one tall grass/flower is generated.

1. Create a new Flat world

2. Use bone meal on the grass blocks

Observed Results:
Only one tall grass or flower is generated.

Expected Results:
Multiple tall grass and flowers should generate.
