This has happened twice, once with my friend and once with me. We died in the nether and respawned, but when we went back through the nether portal, instead of being taken to our normal portal, we were sent to a circular room where our nether coordinates matched our overworld coordinates from our portal, a 1:1 instead of 1:8 ratio. There is no portal to get back.

I have also had this problem, and it’s not only after dying, I just had it happen and I hadn’t died yet in that world. The issue has happened in multiple worlds (one world I was stuck 15000+ blocks from my base). It doesn’t appear to matter how many portals you have, I’ve had it happen with lots and just now with only one portal. And there doesn’t seem to be any reason why it happens, it’s completely random.
