My minecraft is broke

I play on the Xbox one. Lately on Minecraft it's been freezing or glitching up on me and will only unfreeze it self if I press the home button (so that the side screen pops up). It doesn't do it at first, I get to play for like 15-20 minutes then it starts freezing/glitching on me. It does it when I play alone and on other peoples' servers. But when it does it, it seems like it's just lagging but then it freezes and I can't move anything until I press the home button on my controller. And even then it will freeze again and I have to do it many times. I've tried restarting my game and joining back into the world but after another 20 minutes it'll start again. I restarted my Xbox multiple times and reinstalled Minecraft multiple times to see if it would change but it hasn't. I even cleared all the cache on my Xbox and in game, I also deleted some of my words to see if storage was an issue but still no hope. I really enjoy playing this game I just want to play it again. Is there anything you can do ?
