Long time world 4+ years. 400mb in size. (now 950mb!)
Most recently, since 1.18 and definitely now in 1.19 the performance of the world has gotten worse.
such as hoppers taking ages to pass through items, digging with haste2/eff 5 causing the blocks to disappear and then reappear to have to be dug again. The main performance issue is when traveling from nether to overworld whole blocks of area are missing. You can see right through them to the other side. Cannot pass through them. If I wait five minutes or so they, most of the time sort themselves out however the recent trend is for the game to crash entirely when passing through a (very slow to react) portal. There have been a few occasions where on passing through I seem to get stuck in a block and die losing all gear which is frustrating.

This also is happening on the PC version. I use realms to transfer my PlayStation world to the PC bedrock version and got the same "block/chunks" missing. The delay was less however my laptop is quite a bit faster than the PS4 so it must be something more fundamental.
When running the same world on realms it seems to behave a lot better. Have not experienced the missing block issue yet in realms - it seems to load the chunks differently/smoother?
Something very odd with file sizes though. On the PS4 it is 950mb. On the laptop only 500mb. Could not see anything missing after traveling around a good part of my world...Even when I transferred the 500mb world back to realms and then to PS4, it became 950mb again on PS4.

Exact same issue on Xbox One. I'm surprised mojang released this 1.19 version as the stable release. The lag happens every 5 to 16 seconds randomly, the best indication is the game will go silent, all mobs and Redstone freezes, chests don't open, then about 1-2 seconds later, in a burst of actions, the game will catch up with the actions made during the buffer overrun.
Sometimes, blocks are lost, sometimes the player takes fall damage while pilaring up, sometimes blocs on the ground will not be picked up.
I too have a 3 year old world and 1.18 had similar issues just not this bad.
