When sprinting across farmland and holding down the "place" button, you periodically take seemingly random damage. I have a 100% secure multilevel underground farm, and when it happened on one level, I checked below it and found a square had reverted back to regular dirt, and the crops were next to it as if I had fallen on it. I took a video clip.

The top layer of the farm is open air, and is where I first encountered the problem. I found that it happens when walking and planting too, not just sprinting. Also, when walking across scaffolding, you dip down ever so slightly instead of it being perfectly level with the ground. To further indicate that it's fall damage, I later had this happen: when I "fell" on the layer below, I apparently picked up the beetroots that I had uprooted in the "fall." https://youtu.be/KCi3cGfaEH0
Unfortunately I don't know how to export a world from PS5. It's also a realm, if that makes a difference.

The issue might be caused by the low ceiling. Does the issue still occur if the pumpkins on the ceiling are removed?
Also, could you please export and attach the world in which you experience the issue?
