While in the nether. I had my diamond sword in slot 1, and my iron pickaxe in slot 3. I did some mining, switched to sword to fight. Then trying to retreat, I had used the sword to mine some netherrack or nether quartz to get away. Then suddenly my sword in slot 1 became an iron pickaxe. This happened twice on different occasions, both in the nether. And I still had my iron pickaxe in slot 3. So I ended up with two iron pickaxes and no diamond swords.
FYI
I use city texture pack.
This happened on a realm that I own.

After reading through more bugs. I think this is also related to MCPE-17606, because I was playing on Realm with other people.

I am now able to consistently reproduce this problem. And it is NOT related to the problem mentioned above with feeding. I haven't tried on single player, but this happens on the realms by doing this :
1. Create zombie spawner or something that can keep the mobs coming
2. Have a sword in slot 1, and a (anything) in slot 2, such as a shovel
3. Then attack quickly (clicking at speed)
4. While attacking, use the keyboard to switch between slot 1 and 2
Very quickly one of the two items will duplicate over the other ones.
I will try this in single player as well
