Graphics problems

Not sure this has been reported before, but for whatever reason, seemingly without a cause, the textures of blocks and items get severely unorganized, simplified, or completely disappear. When I open the inventory, there are empty squares that, when pressed, will show the name of the item there, but the texture is invisible. This has happened on multiple occasions and never happens to the same blocks.
